This is a confidential form for residents to report any workplace collective agreement violations.

All information from this form will go directly to Sandi Carew at Maritime Resident Doctors and will be kept confidential. We will compile all the information and proactively work with Programs to better workplace environments. We will not act on any single incident to avoid breaching anyone’s anonymity.
